Our goal is to help make that transition back to home successful by working with the patient, their family, and other care givers to develop an individual plan of care.<\/p>\n As a part of this plan, we will establish contact with the patient within 48 hours after discharge from the hospital. After exchanging the necessary information and providing immediate medical direction, a face-to-face visit will be arranged within 14 days. Her younger years led her to competing in mountain biking and road racing where she earned 5 national titles. These experiences allowed her to travel and live in areas such as Italy, Cypress and Great Britain. \u00a0During this time, she also went to school in Durango Colorado and earned a bachelor\u2019s degree in psychology. Upon obtaining her degree she began her career by moving to Montrose where she went to work for the Center for Mental Health in the behavioral health platform. She worked as a Medical Care Manager for many wonderful Psychiatrists. Moving to Montrose has allowed her to not only be with the love of her life and best friend but allowed her to be in touch with what she truly loves outside of work such as camping, riding her dirt bike, hiking and relaxing in the summer evenings. \u00a0In 2018, she was awarded the opportunity to take on a new endeavor as lead of the Care Management program with Cedar Point Health.
